Vendor Requirements
- Complete Registration Form
- Upload a certificate of insurance with the below requirements
- Upload a completed W-9
- Upload any licenses
- Upload any standardized pricing lists
Insurance Requirements
- Certificate must name the following as additional insured:
- General Liability Minimum – $1,000,000
- Automotive Liability Minimum – $1,000,000 combined single limit
- Worker’s Compensation Insurance – $1,000,000/$1,000,00/$1,000,000
- Waiver of Subrogation to be included
